Source text - English Although initially believed to cause diarrhea in only patients with ileal Crohn’s disease or ileal resection,[19–21] bile acid malabsorption (BAM) is now known to be present in up to 50% of patients with IBS-D. It remains unclear whether BAM in this population is the primary cause of IBS-D symptoms or the by-product of rapid transit through the small intestine. Bile acids in the colon are thought to induce diarrhea through multiple mechanisms including an increase in mucosal permeability,[22,23] water and electrolyte secre-tion through activation of cystic fibrosis transmembrane conductance regulator via adenylate cyclase [24] and by inhibiting apical chloride–hydroxide exchange,[25] mucous secretion [24,26] and acceleration in colonic motility.[27,28]
Currently in the United States, there are no clinically available tests to diagnose bile acid diarrhea. However, else-where in the world, tests such as SeHCAT are widely avail-able.[28–30] During the SeHCAT test, the patient ingests a capsule of synthetic bile acid with radionuclide tracer. Translation - Russian Несмотря на первоначальное мнение о том, что мальабсорбция желчных кислот (ЖК) приводит к диарее только у пациентов с болезнью Крона подвздошной кишки или резекцией подвздошной кишки [19-21], на сегодняшний день известно, что она встречается у вплоть до 50% пациентов с СРК-Д. Остается неясным, является ли мальабсорбция ЖК в данной популяции основной причиной развития симптоматики СРК-Д или следствием ускоренного транзита через тонкий кишечник. Предполагается, что желчные кислоты в толстом кишечнике приводят к развитию диареи путем различных механизмов, включающих повышение проницаемости слизистой оболочки [22, 23], секрецию воды и электролитов в результате активации муковисцидозного трансмембранного регулятора проводимости под действием аденилатциклазы [24] и ингибирования обмена йонов хлорида и гидроксида на апикальной мембране [25], секрецию слизи [24, 26] и усиление перистальтики толстого кишечника [27, 28].
В настоящее время в Соединенных Штатах в клинической практике отсутствуют диагностические тесты для определения хологенной диареи. Однако в других странах мира широко распространены такие тесты как SeHCAT [28-30]. В ходе SeHCAT-теста пациенту предлагается проглотить капсулу с синтетической желчной кислотой, меченной радиоактивным изотопом. Далее изотоп отслеживают при сканировании во время пищеварения и спустя 1 неделю. Низкий процент задержки желчной кислоты в организме (< 15%) указывает на мальабсорбцию ЖК [31].
