All patients participating in the EAP will be provided an informed consent form describing the EAP and providing sufficient information for patients to make an informed decision about their participation in this program.
The EAP Informed Consent Form will be submitted with the Program Guidelines for review and approval by the EC/IRB for the program.
The formal consent of a patient, using the EC/IRB-approved consent form, must be obtained before that patient is submitted to any program procedure.
The Informed Consent Form must be signed by the patient or legally acceptable surrogate, and the Treating Physician-designated research professional obtaining the consent.
All patients will have adequate time to ask questions and will be provided with a signed copy of the consent for his/her records.
A confirmation of documented consent will need to be attested to, but copies of the actual consent form do not need to be submitted to WEP Clinical or Rezolute.
Safety
Pharmacovigilance monitoring (SAE collection and reporting) will follow recommendations outlined in the International Conference on Harmonization (ICH) GCP and the protection of human safety.
The Treating Physician should follow standard of practice care for infusion therapy and safety evaluations (vitals, infusion reaction, local lab collection, *serum/point-of-care (POC) ketone values*, concomitant medication review, etc.).
