Have you skipped any meals? - is even better, I agree.
Peer comment(s):

agree Sophia Hundt (X) : Is it OK to agree with two answers? 'Skip' is very good too.

4 KudoZ points awarded for this answer. Comment: "Sounds the most "English", especially with "any" added."

Did you follow the diet instruction? (including no missed meals)

Похоже на обратный перевод.
Я переводил с английского в этом году следующее:
Did you follow the diet instruction? (including no missed meals)

Правда, допустил сначала ошибку (спутал приемы пищи с блюдами), и редактор меня поправил (и правильно сделал).

Have you ever missed your meals?

Have you ever missed your meals sometimes?

Ever, наверное, не обязательно, Have you missed your meals? или Did you skip your meals?, но YOUR перед meals, мне кажется, напрашивается и требуется.
Peer comment(s):

agree Alla Tatyants : Have you missed your meals?-согласна с этим вариантом,вот только напрашивается вопросительно-отрицательная форма- Haven't you missed...(IMHO) :)

Я вот боюсь, что при таких наворотах трудно будет понять, "да" или "нет" в опроснике. "Вы уже перестали пить коньяк по утрам?" ;)
neutral Sophia Hundt (X) : comment for Alla's suggestion: I wouldn't say "haven't you" cause it suggests that you are sure that the patient missed some meals.

neutral Vladimir Dubisskiy : Found your "fresh" answer in the Comments of Sophia to Cortejo. Also with your added '..ever..' the relevant answer can easily be: "yes, three years ago" :-)) then 'ever' and 'sometimes' together - the English is just not good here (to start with :-))

Whether or not you missed your meals.

i'd put it this way.

--------------------------------------------------
Note added at 1 hr (2006-08-22 19:58:24 GMT)
--------------------------------------------------

like:

Did you follow your diet instruction?
(and whether or not you missed your meals).

and (i found it interesting):

Is "Whether or not" a good grammar?

or even simpler:

Did you follow your diet instruction?
(or whether you missed your meals).
Peer comment(s):

disagree Kirill Semenov : "Whether or not" is not good here, because you can't answer "Yes" or "No". Otherwise your answer is repetition of everything proposed above

Not fair. None used this constrcution which is absolutely correct here as it's 100% fits the original (...ли). You're wrong here.
