Date of issue of policy

You can't talk about the "seniority" of an insurance policy.

This expresses it slightly differently from the Spanish; it would be followed by a date without "since". But it is what would normally appear on a form in English, I think. Another possibility is "Policy held since:".

"En el ámbito asegurador se habla de «la antigüedad de una póliza» para referirse a los años que lleva contratada en la compañía de seguros."

"How long has the policy been held?" is another approach, which gets the meaning across, but I don't think it would be so likely to appear on a real form in English.

Commencement or start-date of insurance

Might be different from the date: 'expedido' - see the weblink, namely issued in March to start on April Fool's Day.
