Yes. The above example comes from an official school form. I have no problem with it at all. However, in the context provided by the asker we are dealing with an everyday informal school conversation. I would stick to the "unprepared options".

Most likely the teacher would say "this is third time you came unprepared". But I see it also possible for him to utter "I am making an entry into the journal for your unpreparedness"

We don't have to agree. It is a free world. Would a teacher tell his/her student, "This is your third unpreparedness"? Hmmm...

I agree with Dariusz that on the college or university level "unpreparedness" does not exist - who cares? But I have seen it used in elementary, junior high and high school levels.

I posted few examples in the context of "class unpreparedness" - see my entry. As per "unnatural" ringing to it, I am on complete opposite - it sounds to me natural as I have seen it used in this context, e.g. "# of times unprepared" "second unpreparedness" "reasons for unpreparedness" etc.
I can offer my solution as a compromise.:)

W tej konkretnej sytuacji jestem za różnymi opcjami z "not prepared" "unprepared": be unprepared, show up unprepared, come to class unprepared etc.

Ja chodziłem do szkoły tu i uczę w szkole (college level) od 20 lat i w codziennej konwersacji zabrzmiałoby mi to dziwnie.
