This is taken from a medical report on surgery on a cancer patient. During surgery tumour samples were taken and, as it describes:
"Se toman muestras para congelación I.P"
Further references are:
"múltiples áreas nodulares de superficie blanquecina con punteado amarillento de aspecto necrótico, el mayor de 9,5x2,8x1,3 cm. I.P"
"Se toman fotografías macroscópicas. I.P."
Later examples use "IT":

To me this is the most informative bit here:

As you say, fotografías (de la) "immunoperoxidasa" does not make much sense. Fotografías macroscópicas "de la inclusión parcial" does not seem to work here either (I believe). Maybe (fotografías macroscópicas) "de las inclusiones parciales"?. Not sure about this. That is why a reference to the professional involved seems (to me) more likely. In Spain, a personal acronym would more naturally involve three and not two letters, which is the reason why I tend to think IP/IT might inform about pathologist vs. technician, for example. But as you say, acronyms in this context are used in a rather original way, so it is only the specific original context which might help dilucidate the particular meaning of the acronyms.
She said they use ¨entrada parcial¨and ¨entrada total¨ and that here the ¨T¨ was precisely linked to the smallest sample in this case . The ¨I¨ could be ¨inclusión¨ (inclusión total, inclusión parcial).
However she reminded me acronyms used by laboratories are not standard and each one has their own way of identifying how they process samples at a macroscopic level and there are wide differences even within the same country.
