The patient is conscious, alert, oriented to time, place, and person, with proper eye to eye contact, and grandiose attitude. Speech is of normal volume and rate, occasionally not to the point. The patient is irritable, easily provoked. Formal thought disorder in the form of offpointing and occasional derailment. The patient has delusion of persecution that unknown people want to harm him, delusion of being followed, delusion of grandiosity and auditory hallucination in the form of voices calling his name. Patient is insightless.
