Herida que queda en la piel y en las partes blandas por necrosis, formando una costra seca. Normalmente es consecuencia de una quemadura, una infección o una enfermedad cutánea escoriativa, resultado del apoyo prolongado sobre una zona determinada en pacientes encamados. En este último caso, se denomina también úlcera por presión o úlcera de decúbito.

En el contexto específico del original, "escara sacra grado 4" hace referencia específica a una úlcera por presión/decúbito de grado 4 (stage 4 decubitus ulcer). De ahí la propuesta de traducción, evitando el innecesario literal "eschar" en este caso.
