agreement With due respect. A "report" retells and summarises other people's words. In a courtroom, it's important for a magistrate to hear that the two parties exressly agree to reconcile. https://www.law.cornell.edu/cfr/text/24/103.310
Also, a "reconciliation report" is an a term in accounting http://help.acclivitysoftware.com/us/ab04/rba0103.337.1.htm. This however does not have to go to a court.

reconciliation agreements/statements that are certified by the courts

1. As Milena pointed out earlier, these are not reports. Parties in conflict AGREE to reconcile and do so either by declaring or STATING so orally or by committing the terms of their reconciliation to paper. Therefore, we rather call them AGREEMENTS or STATEMENTS.
2. The entry at ProZ is undocumented despite the peer support it received.
3. CERTIFIED and not APPROVED is the meaning sought here for, a court certifies a reconciliation agreement/statement to validate it legally for the record; it does not approve or disapprove it or of it.
