: Creased means "with a lot of deep/profound wrinkles", i.e. age lines. "Smyato", I think refers more to being tired or in whatever _temporary_ state.

"Otechnoe litso" means that he has some puffy face (prob. kidney failure or some mild heart condition). If you take, say, an untouched "puffy" pillow and put some pressure on it with your fist / palm, etc. - the pillow surface will become "smiato".
