Hola,
se trata de un documento español del registro mercantil.

Dentro de este apartado se indica el número de factura, el número de entrada y luego un cuadro cuyos encabezados son: bases, arancel, conceptos, cantidad, honorarios y total, indicando al final lo que ha costado (supongo) todo el trámite del registro de la empresa.

¿Sería una simple "note" o "draft" o "memorando" (Hughes) o bien una "bill" o "fee"?

Es el término que yo recomendaría, ya que arancel=tariff/duty, honorarios=fees, etc. y deberíamos usar un término neutral.
