I am translating a short series of comments from physicians about improvements they would like to see in the treatment of cancer (unspecified). Not sure what "a pie" means here, does it mean "on-site"??

Emma and Lina have explained "de a pie" - I think this means in contrast to the head of department, who might only be consulted for special cases, although of course all the cases are special.
And I take cercanía to be the ability to be seen by, get hold of, accessibility in that sense

: I think so; I imagine this as referring to the ordinary footsoldier as opposed to the officer class on horseback. / Just checked the DRAE; that is the origin, and by extension "normal y corriente", as Lina says

: Yo también. Parecería que los médicos sugieren asignar un oncólogo que esté cerca del paciente para poder examinarlo más frecuentemente. O que el paciente pueda acudir a su consultorio fácilmente. Saludos Mónica.

As already mentioned "de a pie" means "average, ordinary, general" . This I suppose is as opposed to an "expert" oncologist, such as in lung cancer, breast cancer, leukaemi, etc

El original es incorrecto; la expresión es " a pie" no "de a pie", y la estructura de la frase es muy extraña, pero 'within walking distance" es como se diría en inglés. Saludos.

Por un lado "de a pie" no tiene nada que ver con andar. Quizá etimologicamente, los que andaban vs los que iban a caballo, pero hoy significa "normal y corriente". "El ciudadano de a pie" es "ordinary citizen". Por otro lado, la "cercanía" no se refiere a distancia física sino a relación cercana.
El comentario establece una comparación entre los oncólogos que tratan con los pacientes, "oncólogos de a pie", los que se enfrentan al cáncer en la vida real, con los investigadores del cancer que están es sus laboratorios y son los que diseñan los tratamientos del cáncer, pero no tienen relación con el cáncer en la vida real.
En la "discussion" hay dos o tres notas mías (qué pesada). Una es una carta a Mariano Barbacid, el investigador de cáncer español más importante, dierctor del Centro Nacional de Investigaciones Oncológicas, etc, en la que otros oncólogos se quejan de que los investigadores están lejos de la realidad de los paciente de cáncer, que es una realidad que conocen los oncólogos "de a pie", los "practising oncologists".
