I am translating a medical report for a cardiology patient, and the heading contains the entry "NHC SMS:" with nothing filled in. From previous KudoZ questions, I've been able to glean that NHC is medical record number, but what about SMS? Thanks in advance for any insights.

On page 13 the NHC SMS is a number of the form CARMXXXXXXXXXXXX (where X is a number) which looks very much like an NHC from Comunidad Autónoma de la Región de Murcia (see https://www.laopiniondemurcia.es/comunidad/2022/10/13/pedir-...
The next entry on the image is the "CIP CARM" which is the Código de Identificación Personal (again relating to Comunidad Autónoma de la Región de Murcia)
I can find no other references to 'NHC SMS'. However, as you pointed out, this is from a hospital in Murcia, so it's a possibility.
