If the patients 'se derivan espontáneamente', does this not suggest 'patients turn up without an apppointment' rather than being referred? Surely the verb for 'refer' is 'mandar'.

: as in the second sentence is "pueden concurrir tambien al...", I think that derivarse here means turn up.
